Welcome to the DeKalb County Health Department's Vital Records online ordering system. Requests will be processed Monday - Friday: 8:30 AM - 4:15 PM. The Health Department processes orders within 2 business days, excluding weekends and holidays, once all information is available.  Proper identification must be provided to request a birth certificate, and only immediate family may apply.  A person who has no form of ID should have an immediate family member apply for them.  Family members must be over 18 years of age and have proof of relationship, as well as personal identification.  Please click here to review ID requirements.

 

Important Note regarding Newborns: Certificates for new births typically take up to 2 weeks to become available.  While the Health Department strives to provide this service as quickly as possible, the information must first be received from the birthing center and uploaded into the State of Indiana records system.

 

For all death records outside of DeKalb County you may contact the specific county or the Indiana State Department of Health in Indianapolis (ISDH).  ISDH no longer provides walk-in service as of December 10, 2010. PLEASE NOTE: For a birth or death between 1882 - 1920, we may not be able to supply complete information. 

 

REQUESTING A BIRTH OR DEATH CERTIFICATE

 

Birth Certificates 

  • A copy of your birth certificate is on file in the County in which you were born.
  • All persons born in DeKalb County after 1882 have written birth certificates on file with the Health Department.
  • We can now process birth certificate requests for anyone born in the state of Indiana, regardless of the county. 

Death Certificates 

  • A certified record of death can only be obtained from the county in which the individual passed away, no matter where he/she was buried.
